Hospitalist Opportunity in St. Louis, MO!

Mercy Hospital St. Louis and Mercy Hospital South are currently seeking a Nurse Practitioner to join the Hospital-at-Home program in St. Louis. Missouri. As part of Mercy's mission to transform the care experience, last year we launched the Hospital-at-Home (HaH) program, in partnership with Maribel Health. The role of the Advanced Practitioner will be in the hospital setting screening patients who qualify for this new, innovative program.

Patients who have presented to the Emergency Department and have acute care needs that require hospitalization are thoroughly screened for admittance to the HaH Program. Once "admitted", these patients are transported home to receive all the acute care services during the typical 3-5-day admission. This includes twice daily nursing visits, medical direction oversight, medication management, equipment, and all additional program support.
