Job Description

The Salvation Army Ferguson Community Empowerment Center is looking for a Community Pathway of Hope Case Manager. This individual will work with families referred internally or by other service agencies and community partners.

 

This is a full-time, 40 hours per week, $44, 000 per year. This is a Grant funded 3-year position.

Essential Functions

  1. Follow the Pathway of Hope approach consisting of the following functions with a limited number of participating clients:
    1. Conduct pre-screening and intake of clients using specified assessment tools.
    2. Conduct goal setting steps with clients formulating a change-oriented action plan.
    3. Conduct follow-up case management meetings with clients.
    4. Monitor and track the changes and goal attainment on the action plan.
    5. Provide persons seeking assistance with intake and assessment activities to determine client eligibility and to match clients with resources. Provide persons seeking assistance with intake and assessment activities to determine client eligibility and to match clients with resources.
    6. Identify and secure housing options, programs and services for clients identified as homeless or housing insecure.
    7. Provide on-site, remote, community case management to clients seeking emergency assistance.
    8. Conduct community education awareness opportunities as needed by the population served.
    9. Maintain client files and complete monthly statistical reports.
  2. Distribute food, vouchers, and other material assistance as available to clients for purchases and payments and refer persons to other agencies if assistance is unavailable.
  3. Identify resources available in-service areas and maintain a current community resource guide to provide information and referral services and inform persons of the resources available to them.
  4. Network with community agencies and organizations to maximize services to persons and occasionally liaison between clients, The Salvation Army, and other agencies.
  5. Record electronic and written client information in paper and computer records and maintain client confidentiality.
  6. Case Manager will expend funding and in-kind gifts in a manner that conforms to spending deadlines and utilizes tracking methods prescribed by Salvation Army policy. All reports must be submitted in a timely manner.
  7. Direct, schedule, and supervise the volunteers helping with Pathway of Hope families.
  8. Communicate Pathway of Hope activities to the supervisor, coordinate social services with other Corps programming, and inform clients of Corps programming.
  9. Communicate to stake holders, providing status updates as needed.
  10. Submit required reports as needed.
  11. Facilitate client application process for seasonal and special assistance as well as community programs.
  12. Perform other duties as assigned (e.g., in-service training, fund-raising/special events, disaster or seasonal activities.
  1. Represent The Salvation Army through the implementation of its mission in all service aspects.
  2. Maintain confidentiality and comply with Safe From Harm guidelines.

 

Education and Experience

 

Bachelor’s degree in human services field or at least (3) years related work experience.  Must have an outgoing personality with good communication skills and an ability to work with diverse cultures.  Strong leadership style and good organizational skills required.  Commitment to carry out The Salvation Army’s mission is essential.  Knowledge of The Salvation Army and Government, State and community resources is helpful.  Must have working knowledge of computers.  Must have reliable transportation, a valid Driver’s License, and pass TSA MVR check.  Good communication skills as well as the ability to work with diverse and challenging personalities are essential.

 

 

Physical and Work Environment Requirements

Capable of performing in an office environment to include using a computer, answering phones, moving client files, etc.  Most work will be indoors in a temperature-controlled environment.
